Documents You Need for Your Passport Appointment: The Complete Checklist

You spent days, maybe weeks, getting your RPO appointment. The last thing you want is to show up and get turned away because you forgot a document. It happens more often than you would think, and the passport office will not make exceptions. If your paperwork is incomplete, you go back to square one.

This checklist covers every document you might need based on your specific situation. Bookmark this page and go through it the night before your appointment.

Documents for a Fresh Passport (Adults)

If you are applying for a passport for the first time, bring the following:

Proof of Date of Birth (Any One)

  • Birth certificate issued by the Municipal Corporation or District Office
  • Aadhaar card or e-Aadhaar
  • PAN card
  • Voter ID card
  • Transfer or school leaving certificate with date of birth
  • Class 10 mark sheet or certificate from a recognized board
  • Policy bond issued by public life insurance corporations with date of birth

Proof of Address (Any One)

  • Aadhaar card or e-Aadhaar
  • Voter ID card
  • Water bill, telephone (landline or postpaid mobile), electricity bill, or gas connection bill in the applicant's name (not older than 3 months)
  • Income tax assessment order
  • Bank account passbook with photo (latest 3 months statement)
  • Rental agreement (registered) along with one utility bill of the landlord
  • Spouse's passport with current address (if applicable)

Other Required Documents

  • Printed appointment confirmation from the Passport Seva portal
  • Two recent passport-size photographs (white background, 3.5 cm x 3.5 cm)
  • Aadhaar card (original and one photocopy)
  • Self-declaration form (Annexure D for non-ECR category, if applicable)

Documents for Passport Renewal (Re-issue)

Renewing an existing passport is generally faster than a fresh application, provided your documents are in order:

  • Old passport (original and photocopy of first two and last two pages, ECR/non-ECR page, and any observation page)
  • Aadhaar card (original and photocopy)
  • One proof of address (same options as fresh passport)
  • Printed appointment confirmation
  • Two passport-size photographs
  • Self-addressed Speed Post envelope (some RPOs require this)

If you are renewing because your passport is damaged, carry the damaged passport. If you are reporting a lost passport, bring a copy of the FIR or police complaint and an affidavit on non-judicial stamp paper.

Documents for Name Change or Correction

If you are re-issuing your passport due to a name change (marriage, divorce, legal name change), you will need additional documentation:

  • Old passport with the previous name
  • Marriage certificate (for name change after marriage, issued by the Registrar of Marriages)
  • Gazette notification (for other name changes, published in the Official Gazette)
  • Deed poll or sworn affidavit for the name change
  • Newspaper advertisement of the name change (some RPOs require this)
  • Updated Aadhaar card reflecting the new name

Important: Your Aadhaar card should reflect your new name before your RPO visit. If your Aadhaar still shows your old name, the verification officer may ask you to update it first.

Documents for Minor Passport (Below 18 Years)

Applying for a child's passport involves additional paperwork because both parents need to provide consent:

  • Birth certificate of the child (original and photocopy)
  • Aadhaar card of the child (if available)
  • Both parents' passports (original and photocopies) or Aadhaar cards
  • Annexure D (declaration of parents, signed by both parents)
  • School ID or bonafide certificate (for children aged 5 and above)
  • Printed appointment confirmation
  • Two passport-size photographs of the child

Both parents should ideally be present at the RPO. If one parent cannot attend, they must provide a notarized authorization letter along with a photocopy of their ID. In cases of single parents, an Annexure C (declaration of sole parent) is required along with supporting documentation.

Documents for Tatkal Passport

Tatkal passport applications follow the same document requirements as regular applications, with one addition. You need to provide one of the following verification documents:

  • Annexure F: Identity certificate issued by an officer of a scheduled commercial bank (on the bank's letterhead with the officer's name, designation, and employee number)
  • Annexure I: Verification certificate issued by a serving Group A gazetted officer or an elected member of a legislative body on their official letterhead

Getting this annexure is often the trickiest part of a tatkal application. The bank officer must personally know you, and not every branch will agree to issue the certificate. Start working on this as soon as you decide to go the tatkal route.

Tips for Your Appointment Day

Beyond the documents themselves, here are a few things that will make your RPO visit go smoother:

  • Carry originals of everything. Photocopies are for submission. The officer will verify against the originals and hand them back to you.
  • Arrange documents in order. Keep them in the same sequence as the application form. This speeds up verification at Counter A.
  • Dress for your photograph. Your photo will be taken at the RPO for the passport. Wear a collared shirt or professional attire. Avoid white clothing as the background is white.
  • Arrive early. Get to the passport office 15 to 20 minutes before your slot. Late arrivals may lose their appointment.
  • Keep your phone charged. You may need to show your digital Aadhaar or appointment confirmation if your printout is unclear.
  • Do not carry large bags or extra electronics. Many RPOs restrict what you can bring inside and do not have locker facilities.

What Happens If You Forget a Document?

If you arrive at the RPO and the verification officer finds that a required document is missing, your appointment will typically be cancelled. You will need to rebook and come back another day with the complete set of documents.

This is why preparation matters. Going through this checklist the night before your appointment can save you from wasting a hard-earned slot and going through the booking process all over again.
